Diagnosis and therapeutic management of primary headache in an emergency setting

Introduction and aim: Much headachers are under or mis-diagnoses and data regarding the proportion of patients attending an emergency department (ED) because of headache are still few. We conducted a retrospective observational study in an ED with the following aims: (a) estimate the proportion of headache attending to an ED (b) to estimate and describe the therapeutic management of primary headache and (c) to assessment the exam most frequently requested. Materials and methods: We collected data regarding patients diagnosed with headache consecutively attending the ED of the University of Palermo between September 2011 and March 2012. The study was approved by the ethics committee. Results: Between the semester evaluated 25110 subjects were admitted to ED headache suffers were equal to 1.6 %. Of these 263 (63.1 %) were woman and 154 (36.9 %). Mean age was 44.2 (DS ± 18.4) years (p = 0.068).According to ED registry headache admission was as follow assigned: 76.5 % with a diagnosis of headache 22.8 % with a secondary headache 0.7 % with Trigeminal Autonomic Cephalgias (TACs). Among those with a primary headache about 36 % of patient did not received a pharmacological treatment. Monotherapy was prescribed less frequently than combination therapy (19.1 vs 44.5 %).In monotherapy the most frequent medication were NSAIDs (28.3 %) benzodiazepines (26.7 %) and dopamine antagonists (11.7 %). Among those with a primary headache a CT scan was performed in the 124 subjects and 111 (34.8 %) had a neurologist consultation. Discussion: Our data are in line with the one previously reported in literature. The most frequently medication in the Italian ED were NSAIDs benzodiazepines dopamine antagonists and steroids. Neverless our data unlikely can be compared to other study give a snapshot. We believe that much more can be done to improve treatment of primary headache in ED.Settore MED/26 - Neurologia

New technologies have radically transformed not only geography as a discipline but also the geographies of the world. Indeed the digital has assumed an increasingly pervasive mediating role in the production of space as of geographic knowledge and imaginaries. The possibilities offered by the virtual in particular now offer a decisive contribution in communicating the meanings attributed to places and respond increasingly effectively to the emerging needs of the tourism sector. Digitally mediated visitor experiences aim to engage the senses and are effective when they promote rigorously realistic immersions. Within the framework of these reflections the paper highlights the renewed relevance of authenticity both of the tourism experience and of cultural heritage in relation to its potential for attraction and proposes a contextualization of the theme in light of the unprecedented opportunities granted by technologies for the creation of virtual and augmented spaces. The proposed discussion is therefore also a journey aimed at exploring authenticity as a tool through which new technologies can effectively mediate a sense of place and foster brand communication of a destination. The case study is the UNESCO site of Tarragona whose most relevant image for urban policies is presented. This shared figuration spatially defined and rooted in a precise historical moment is reconstructed on the one hand through the analysis of functional documentation for the candidacy and on the other by means of a review of mobile applications developed for the promotion of the city's cultural heritage. The proposed journey and exploration of the place return in light of the research data and in conclusion the central role played by digital technologies in mediating the UNESCO imaginary. Design and analysis of discrete choice experiments for models with response time

Settore SECS-S/02 - Statistica Per La Ricerca Sperimentale E TecnologicaA sector of conjoint analysis (experimental design in marketing research) is made of the so called choice experiments. In choice experiments respondents undergo a questionnaire which is nowadays mostly submitted through the internet. The questionnaire proposes to the respondent a sequence of choice sets each one including two or more profiles being a profile a specific combination of attribute levels. The respondent selects the preferred profile for each choice set. Responses given by a sample of respondents are analysed through suitable methods aimed to eventually find the best combination of attribute levels. One method of analysis adopts the Multinomial Logit (MLN) model. In this article the authors show the results of the MLN analysis compared with another model of analysis which uses an additional response which can be easily recorded by electronically submitted questionnaires. In practice modern survey platforms like “Qualtrics” (the one used for this work) can record the so called “response latency” i.e. the time taken by the respondent to make the choice and select the most preferred profile in the choice set. Thanks to a response latency model further refined in this work it is possible to deduce the relative weight of importance of the profiles for each choice set and respondent. This type of response can be used in place of the dichotomous choice variable in the MLN model. The two models and methods of analysis are deeply compared and it is critically discussed when it is better to use one or the other method. As a result a more reliable estimate of the optimal profile comes up implying lower risks for new investments and marketing decisions.

The article critically analyzes the Italian regulation of subcontracting in the context of public tenders as recently amended by several legislative measures in compliance with the infringement procedure launched by the European Commission against Italy. According to the Author’s view the current legislative framework – characterized by the almost overall elimination of the previous limits (probably even going beyond what the European Commission requested) – lacks in finding a fair balance between the principle of the maximum participation in the tender on the one hand and the need to prevent and reveal potential collusive conduct on the other. Furthermore this conclusion is supported by the analysis of the regulation of the causes of exclusion from the tender still too timid in envisaging the optional exclusion of the participants due to anticompetitive conduct.

The pandemic crisis has accelerated the diffusion of learning environments characterized by the intensive use of technology. This novelty represents a transversal objective of the professionalization of the university teacher with a particular declination on the pedagogical-didactic-evaluation side as a strategic issue for the development of higher education. This paper presents the results of research conducted with 119 newly recruited university teachers and 20 Department Directors. The aim of the research was on the one hand to detect the perception of newly hired teachers related to their teaching metacognitive and evaluative skills with reference to distance learning to highlight specific characteristics that could have encouraged or hindered adaptation to this new condition. On the other hand through the qualitative research the aim has been to penetrate in depth the challenges posed by the digital revolution at University by carrying out 20 interviews with privileged witnesses who represent on the national scene agents of institutional change.
